Special Diet Requests for School Meals
If your child has special dietary needs, the district requires a Special Diet Statement to be on file to make accommodations.
Roseville Area Schools Nutrition Services will strive to accommodate special diet requests but it is up to the discretion of the district to make food substitutions for individual children who do not have a disability.
Requests for lactose-free milk due to lactose intolerance are the only medical condition that does not require a signed Special Diet Statement. A note signed by a parent will suffice. Please note, juice cannot replace milk because the nutritional content is not the same and is in violation of state and federal rules.
Individualized Education Programs, Anaphylactic Care Plans, and other documents signed by a licensed physician may act as substitutes for a Special Diet Statement if they contain the following:
Please turn in the Special Diet Statement signed by a licensed physician, physician assistant, or advanced practice registered nurse such as a certified nurse practitioner via:
Roseville Area Schools District Center
1251 County Road B2 W
Roseville, MN 55113
Once the Special Diet Statement is received, you may be contacted to review the requirements and set up a plan to ensure your student’s safe meals at their school. Once on file, the form does not need to be completed annually unless your student’s health condition changes.
